Well, atleast this workaround lets me play Custom Mode still without being gimped item-wise. There's been atleast one other person to have this bug from what i witnessed in this forum. So yeah, this has been a persistent bug on Windows 8. Playing a normal run DOES make higher quality items spawn as normal, but as soon as i start a Custom Mode run, it doesn't. The + quality being around 100, ++ around 60 and S-rank around 5. My save file DOES have higher item qualities unlocked, of course. This has been a bug ever since the Beta version was out, and still hasn't been fixed. If i start a Custom Mode run with them enabled, all item quality will be at 0 as if i intentionally disabled them by setting them to 0.
If i leave Custom Mode entirely and go back to the main menu while Item Quality is active, all Item Quality settings (+, ++ and S) will reset to 0. I can set them to a number, then disable them and leave Custom Mode entirely and go to the Main Menu (possibly even play too, didn't test that), and it will remember the number i assigned them upon re-enabling them. I guess it uses the default save settings for if nothing is enabled and used. There is one bypass though: I started a Custom Mode run while disabling Item Quality settings (and to be sure, setting them to their highest numbers before that), and so far i seem to be getting improved items again. Trouble is, actually accessing those two zonesthe.

So far these are the interactions with it. This week’s Fatal Falls expansion adds a crop of weapons and enemies to the game, plus two new biomes. So, as the title says, for some reason Item Quality (making them +, ++ or S) doesn't stick in Custom Mode when enabling and using them.
